Read this carefully before enrolling your child. By checking the parental consent box at enrollment, the parent or legal guardian agrees to this waiver on the child's behalf. This is a binding legal document.

1. Parental / Guardian Consent

I represent that I am the parent or legal guardian of the child being enrolled, that I am at least 18 years old, and that I have full legal authority to consent to my child's participation in the Services and to sign this waiver on the child's behalf.

2. Health & Medical Disclosure

I confirm that my child is in generally good health and able to participate in soccer training. I have disclosed in the enrollment notes any condition, injury, allergy, medication, or special need (including but not limited to asthma, diabetes, heart condition, concussion history, dental work, ADHD, autism-spectrum sensitivities) that may affect my child's safe participation. I will inform the coach of any change to my child's health before each session.

I authorize the coach, in the event of an emergency where I cannot be reached immediately, to seek emergency medical care for my child and to share relevant disclosed health information with first responders. I am financially responsible for any emergency medical treatment provided to my child.

3. Not Medical Advice

The coach is a youth soccer instructor and is not a licensed physician, athletic trainer, physical therapist, or psychologist. Any guidance provided — including warm-ups, technique cues, return-to-play suggestions, and nutrition tips — is for general educational purposes only and is not medical advice. It is not a diagnosis or treatment for any condition.

4. Inherent Risks of Soccer Training

I understand that soccer training and youth athletics involve a real and inherent risk of physical injury, illness, or in rare cases death, including but not limited to:

  • Muscle strains, ligament sprains, joint dislocations.
  • Bone fractures and stress injuries.
  • Player-to-player and player-to-ball collisions.
  • Concussion or other head, face, or dental injury.
  • Heat illness, dehydration, hypoglycemia, sunburn.
  • Aggravation of pre-existing injuries or conditions.
  • Property loss or damage at the training location.

I voluntarily accept all such risks, both known and unknown, on behalf of myself and my child, even where arising out of the coach's ordinary negligence, to the maximum extent permitted by California law.

5. Release & Waiver of Liability

To the fullest extent permitted by law, I, on behalf of myself, my child, and our heirs, executors, administrators, and assigns, hereby release, waive, discharge, and covenant not to sue Elite Soccer Academy, its owner, coaches, employees, volunteers, and agents from any and all liability, claims, demands, actions, and causes of action arising out of or related to any loss, damage, or injury, including death, that my child or I may sustain as a result of participating in the Services.

This release does not apply to liability arising from gross negligence or intentional misconduct, or to any rights that cannot be waived under California law (which restricts waivers of a parent's right to sue on behalf of a minor in certain narrow circumstances).

6. Indemnification

I agree to indemnify and hold harmless Elite Soccer Academy and its coaches from any claims, judgments, or expenses (including reasonable attorneys' fees) arising from my child's participation in the Services or my failure to disclose a relevant medical condition.

7. Coach Background-Check Disclosure

All coaches and assistant coaches working directly with minors are required to pass a criminal background check (typically through a national youth-sports background-screening provider) prior to working with players, and to renew that screening periodically. Coaches are also expected to complete current SafeSport-style youth-protection training where applicable. Background screening is a safeguard, not a guarantee. Parents are encouraged to remain present during sessions whenever possible and to report any concerning behavior immediately.

8. Photo / Video Consent

I understand that the academy may take photographs and short videos at training and events for promotional and operational use as described in the Privacy Policy. I may opt my child out at any time by emailing the contact address in the Privacy Policy.

9. Pickup & Authorized Adults

The parent or legal guardian is responsible for ensuring the child arrives safely, is picked up promptly at session end, and that anyone else picking the child up is identified to the coach in advance.

10. Severability

If any portion of this waiver is held to be unenforceable, the remaining provisions shall continue in full force and effect.

By checking the parental-consent box at enrollment, I confirm that I have read, understood, and voluntarily agreed to this Liability Waiver, Health Disclaimer & Parental Consent on behalf of myself and my child.
